Camp Registration for the 2017 camps will be available soon.

 

When you open the form page, it will look like this:

Remember to fill out the entire form.  If you forget information, the form will not go to the PayPal payment page and you will NOT BE REGISTERED. Fill in all of the  information and keep following the instructions if you have any trouble. Here is an example of a completed form below:

Most of the form is basic contact information, but there are a few tricky components. The bottom half of the form (shown above in the image to the right) has JERSEY SIZE, PICK YOUR CAMP, and the WAIVER FORM. I will discuss in depth (shown in the image below).

Jersey Size

This portion of the form has a few choices. You will need to select your son/daughter’s jersey size.  Click one of the boxes with your choice.  The options range from Youth Large to Men’s Extra Large.

Picking Your Camp

You may select the Florida or Chicago Camp.

Waiver Form

This is the waiver form. This contains liability information that you must read and sign off on. Please read the agreement and check the box to agree with the terms.

**Remember you need to finish the form and fill in all of the information.  If you do fill out the entire form, it will not send. Once you fill in the entire form, you must click send message.  It will bring you to the PayPal page for payment.  YOU MUST PAY FOR THE CAMP TO FINISH REGISTRATION.


Payment

Depending on the camp you choose, the payment option will be different.  You will need to sign in or create a PayPal account to pay for the camp.  You will need to print off the confirmation page that appears after signing in and confirming your purchase.

***REMEMBER TO PRINT OFF THE FORM TO FINISH REGISTERING.  YOU WILL NEED TO BRING THIS CONFIRMATION TO THE CAMP.

Cancellation Policy

If a player is not able to attend, we must be informed at least 10 days prior to the start of camp and a $ 50.00 administration fee will be charged. If less that the required 10 day notice or camp has started, no refund will be given. Top Gun Hockey School retains the right to terminate the stay of any student when it is deemed to be in the best interest of either the student or the school.
